Remote Patient Monitoring (RPM) is the collection of a patient's physiologic data, such as blood pressure, weight, or glucose, through connected devices at home, with the data reviewed by a care team to manage the patient's condition.
Medicare reimburses RPM under separate CPT codes for initial setup and patient education, for supplying the device with data transmitted on a minimum number of days in a 30-day period, and for monthly treatment-management time that includes interactive communication with the patient. Coverage requires an established patient relationship and patient consent.
RPM programs succeed or fail on engagement. Patients must be onboarded and taught to use devices, readings lapse when devices sit unused, and the required interactive communication has to actually happen each month. Each of those is an outreach task, which is why RPM programs often struggle with staff time more than with technology.
