Definition
A warm handoff is a transfer of a patient conversation to another person, such as a nurse or care coordinator, in which the context of the conversation travels with it so the patient does not have to start over.
In clinical settings, a warm handoff traditionally means one team member introducing the patient to another in person. On the phone, it means an automated or human agent transferring a live call, or scheduling a callback, with the reason for the transfer, the answers collected so far, and ideally the audio attached. A cold transfer, by contrast, drops the patient into a queue with no context.
For automated outreach, the handoff is where safety lives. Escalation rules should define which answers require a person, how fast (live within seconds for an urgent red flag, or a follow-up queue for everything else), and who owns each escalation, including after hours. A flagged finding that reaches no one is worse than no program at all.
